Request for Program Completion Evaluation Included with this form is a checklist and instructions. Please read them carefully before filling out the form. Use this form to request credit evaluation regarding progress toward a degree.
HECB Residency Affidavit/Declaration/Certification Effective July 1, 2003, Washington state law changed the definition of "resident student." Some students who are not permanent residents or citizens of the United States may now be eligible for resident student status and pay resident tuition rates when attending public colleges and universities in Washington state. This affidavit/declaration/certification must be completed in order to confirm eligibility.
Registration Forms
Tuition Refund Request Form Use this form to request a tuition refund in the event a medical emergency or call to active military duty necessitates complete withdrawal from all courses.
Course Repeat Form A student may repeat a course in which he/she has received a failing or low passing grade. If you are repeating a course to rasie your grade, complete this card after you have successfully completed the course to ensure the proper adjustment on your transcript.
Diploma Application Use this form to apply to graduate. Turn in at the registration office at Mount Vernon Campus, South Whidbey Campus and San Juan Center. At Whidbey Island Campus, turn in for at Student Services.
Petition for Reinstatement Use this form when you have been placed on Academic or Financial Aid Drop Status. You will need to meet with your advisor to complete this form.
Hardship Withdrawal The last day to withdraw officially from classes is the end of the sixth week of the quarter unless undue hardship can be established with the approval of the instructor and advisor. Use this form to get Instructor and Advisor approval for hardship withdrawal.

Incomplete Grade Contract Students who have sufficient cause 1) have not completed work necessary to and required by the course, and 2) who discussed the requirements and have made arrangements with the instructor to complete the missing work may use this form to receive an incomplete "I" grade. Instructor Notification Audit Only Card Use this form when requesting permission to audit a course with the Senior Citizen Waiver. Return form with signature from instructor to your local registration office.
Audit Choice Card When requesting to audit a course, take this form to you instructor for approval and signature. Return form to your local registration office.
Waiver Forms
ABE/ESL Waiver/BECA PARA CUBRIR LA COLEGIATURA DE ABE/ESL Students enrolling in Adult Basic Education (ABE) or English as a Second Language(ESL) classes are charged $25.00 per person per quarter. An ABE/ESL tuition waiver may be granted for students with limited income. See form for income requirements
Unemployed/Underemployed Space-Available Waiver Unemployed or Under-employed people may register for pen classes on a space-available basis on or after the first day of the quarter without tuition charges. Fees are still charged. See form for eligibility.
Senior Citizen Waiver People 60 years of age or older, who have lived in Washington State for 12 months or longer, may enroll in a maximum of two clases for a $5.00 tuition fee where there is space available on the first day of the class. Classes are subject to fees. See form for RCW.
