Request for Program Completion Evaluation  Included with this form is a checklist and instructions. Please read them carefully before filling out the form. Use this form to request credit evaluation regarding progress toward a degree.
HECB Residency Affidavit/Declaration/Certification  Effective July 1, 2003, Washington state law changed the definition of "resident student." Some students who are not permanent residents or citizens of the United States may now be eligible for resident student status and pay resident tuition rates when attending public colleges and universities in Washington state. This affidavit/declaration/certification must be completed in order to confirm eligibility.
Immunization Status Form  Use this form to report MMR immunization status. 
Request for ASSET/COMPASS scores, GED transcript, or GED duplicate certificate  Use this form to request you ASSET/COMPASS scores, GED transcript, or GED duplicate certificate.  There is no fee for the ASSET/COMPASS scores.  There is a $4.00 fee for the GED transcript or GED duplicate certificate. 

Waiver Forms

ABE/ESL Waiver/BECA PARA CUBRIR LA COLEGIATURA DE ABE/ESL  Students enrolling in Adult Basic Education (ABE) or English as a Second Language(ESL) classes are charged $25.00 per person per quarter.  An ABE/ESL tuition waiver may be granted for students with limited income.  See form for income requirements
Unemployed/Underemployed Space-Available Waiver  Unemployed or Under-employed people may register for classes on a space-available basis on or after the first day of the quarter without tuition charges.  Fees are still charged.  See form for eligibility. 
Senior Citizen Waiver  People 60 years of age or older, who have lived in Washington State for 12 months or longer, may enroll in a maximum of two clases for a $5.00 tuition fee where there is space available on the first day of the class.  Classes are subject to fees.  See form for RCW.
