SVC H1N1 Flu Clinic

Location: Phillip Tarro Theatre
Time: 11:00 a.m to 2:00 p.m.
Date: Friday, November 13th
Cost: Free

Who is eligible:

  • All students through 24 years of age
  • Household contacts of infants less than 6 months of age
  • Pregnant women
  • All adults age 25-64 with chronic medical conditions
  • Skagit Valley College students and employees only
If you do not fall into one of the above categories you will not recieve a vaccination.
 
Students under age of 18, must print out and bring in a completed parental consent form and a medical/health questions form.
