CARE Team Reporting

What is the CARE Team?

CARE stands for Consultation, Assessment, Resource, and Education.

The SVC CARE Team works directly with students to find solutions to academic, social, and personal issues that may impact a student’s potential for success. Congruently, SVC seeks to foster a climate of safety that requires the involvement of all college community members. Faculty, staff, students, and other community members are encouraged to report observed behavior that warrants concerns for the safety and well-being of the community, or the safety of an individual.

What is the CARE Process?

The CARE process exists to support students as they face a variety of barriers to their success and well-being. By collaborating with the reporter and through partnerships. This could include connecting students to the appropriate department, institutional process, identifying college and/or community resources for assistance.

Anonymous Collection

SVC CARE Team will maintain summaries of CARE reports for information gathering and archival purposes. Through this recordkeeping, the CARE Team will be better equipped to identify any patterns of behavior that exist on campus Records will be securely kept and managed by a tracking tool to assess trends and patterns. Compliance with FERPA and other privacy laws will be maintained.

Confidentiality

SVC is committed to protecting the privacy of all individuals involved in a CARE report. Every effort will be made to protect the confidentiality of individuals targeted and confidentiality requested by the reporting party.

The Life of a CARE Report

The following is an example of steps taken in the CARE Team report process. Some steps may vary depending on the specific details of each case.

  1. Information received by CARE Team Coordinator in one of the following ways:
    1. CARE Team Incident Report
      1. www.skagit.edu/care
    2. Received as follow-up from another incident report (Student Conduct, CVV Conduct, Safety & Security, BIAS, or Title)
  2. Acknowledgement to Reporter
    1. CARE Team Coordinator replies to reporter via email or phone within one business day.
  3. Wait for reporter response (if needed before taking action)
    1. If there is any other follow-up needed before reaching out to the student of concern.
  4. CARE Team Coordinator Outreach to SVC Counseling Department Chair
    1. SVC Counseling Department Chair assigns SVC Counseling Team member to case
      1. Assigned SVC Counseling Team member contacts student of concern.
        1. If no contact made, assigned SVC Counseling Team member continues to reach out via email and phone.
          1. Leaves generic message with contact information.
        2. Assigned SVC Counseling Team member consults with counseling team if needed.
        3. If student of concern does not respond and has not engaged in current classes or other activities, then wellness check may be needed.
        4. If contact made, assigned SVC counselor continues to work with student of concern until they connect the student to appropriate academic or personal support.
        5. Assigned SVC Counseling Team member provides updates as needed to CARE Team Coordinator.
        6. CARE Team Coordinator files case as an FYI report or ongoing updates until case is closed.
  5. Follow-up with Reporter
    1. CARE Team Coordinator follows up with reporter.
      1. Informs reporter that case has been addressed.
      2. Refers reporter for support if needed.
  6. CARE Team Coordinator reviews cases weekly
    1. Receives and documents updates from assigned SVC counselor.
    2. Close any completed cases.

How Do I Report?
Use the “Submit a CARE Report” button below for any academic, social, and personal issues. This report will be reviewed by the CARE Team Coordinator. Any issues requiring additional review will be forwarded to the appropriate campus office.

Who Is On the CARE TEAM?

The CARE Team is the Title IX & Care Team Coordinator, all SVC faculty counselors, and the Vice-President of Student Services. This team meets as needed and the information shared within this group is private. The purpose of sharing CARE reports is to determine how best to coordinate appropriate support to the student.
